I have read and agree to all of the below authorizations & requirements.

FILM RELEASE AUTHORIZATION:
By signing below, I hereby grant [The Original Project Team Foundation, Inc.] ("Producer") the irrevocable right and permission to photograph, film, record, or otherwise capture my likeness, image, voice, and performance in any media format, including but not limited to motion pictures, television, digital media, and still photography (collectively referred to as the "Footage"). I understand and agree to the following terms and conditions:

RELEASE: I hereby release and discharge Producer, its agents, representatives, employees, and all persons acting under its authority from any liability, claims, demands, actions, causes of action, or any legal obligations in connection with the use, exploitation, distribution, and exhibition of the Footage, including but not limited to any claims for defamation, invasion of privacy, or infringement of rights, and I waive any right to inspect or approve the finished product.

OWNERSHIP: I acknowledge and agree that Producer is the sole owner of all rights, including copyright, in the Footage and that I have no claim, right, or interest in the Footage or its use.

COMPENSATION: I understand that I will not be compensated for my participation in the Footage unless otherwise agreed upon in a separate, written agreement.

CONFIDENTIALITY: I will keep any information related to the production of the Footage, including trade secrets, confidential information, and unpublished material, confidential and will not disclose such information to third parties without Producer's prior written consent.

GOVERNING LAW: This agreement shall be governed by and construed in accordance with the laws of [Tennessee], and any disputes will be subject to the exclusive jurisdiction of the courts of [Tennessee].
